ACCURATE COIN GRADING SERVICE SUBMISSION FORM and INSTRUCTIONS Download and print a submission form (Acrobat .pdf - download Acrobat Reader) Accurate Coin Grading Service will evaluate and render an opinion on each coin submitted as to it’s authenticity and will only grade and encapsulate authentic US coins only. (see Coins We Grade) Accurate Coin Grading Service reserves the right to refuse to grade or encapsulate any coin determined ineligible for such services. Due to the time and expertise involved in determining whether a coin is eligible or not, all fees are non-refundable. Accurate Coin Grading Service reserves the right to modify or correct submission forms for appropriate grading fees, return shipping, handling, and insurance fees. Submission Form Instructions 1. Download and print submission form. 2. Fill out your “Return To” address and contact information. 3. Enter Date and Mint Mark for each coin submitted, along with the denomination and major Red Book variety(if any), and owner’s value for each coin. 4. Total the values for all coins submitted and enter that amount on the bottom of the submission form “TOTAL OWNER‘S VALUE“. This is the amount to use to calculate the return Insurance Coverage needed. 5. Place all coins submitted in 2 x 2 Non-PVC flips and mark each flip with the corresponding submission form line number. DO NOT USE STAPLES (they could damage coins during shipment). Flips will not be returned. 6. Enter number of coins submitted and multiply by $15 to calculate total grading fees, and enter that amount at the bottom of the submission form on the appropriate line. 7. Calculate return shipping, handling, and insurance fees using the rate charts below, and enter the correct amount on the appropriate line at the bottom of the submission form. 8. Add totals and enter that amount on the line marked, “TOTAL AMOUNT ENCLOSED”. 9. Sign and date the submission form. 10. Make check or money order payable to Accurate Coin Grading Service. Payment in full must be enclosed with your order. Coins will not be processed without appropriate payment enclosed. 11. Accurate Coin Grading Service will not be responsible for coins damaged during shipment to our facility due to inadequate shipping or packaging protection. Please do not use any staples that could potentially damage coins. To calculate return shipping, please add Shipping & Handling and Insurance coverage based upon the number of coins submitted and owner’s total value. SHIPPING & HANDLING
